Orders may be placed via above email, telephone, or fax. Please include the following information:

1. Purchase order number
2. Name of Institution
3. Shipping address, telephone, and end-user name
4. Billing address, telephone, and contact name
5. Catalog number, description, and quantity
6. Credit card type, account number, and expiration date (if applicable)

Shipping Information

Products are shipped overnight via FedEx. Please store at recommended temperature as listed on technical data sheet upon arrival. There is a $50 shipping charge for domestic shipments, $65 to Alaska, Hawaii, and Puerto Rico, and $150 international that is prepaid and added to invoice, or you may provide a FedEx account number at the time of order. Freight terms are F.O.B. destination.
